We are looking for a dedicated Care Coordinator to join our team and make a meaningful impact on our clients' lives.Role Overview:As a Care Coordinator, you will play a pivotal role in overseeing client admissions, supporting daily routines, and ensuring the smooth operation of our residential facility. You will work closely with the clinical team, facility manager, and clients to deliver exceptional care while fostering a positive and therapeutic environment.To succeed in this role, one must have high level ownership and ability to build relationship with cross functional teams.Key Responsibilities:Coordinate client admissions, including check-in/check-out processes and financial transactions.Support clients in their daily activities, ensuring adherence to program guidelines and promoting participation in therapeutic, educational, and recreational activities.Act as the primary point of contact for client requests and complaints, resolving issues promptly and professionally.Collaborate with clinicians, nurses, and therapists to provide integrated care and support individualized treatment plans.Complete accurate and timely documentation, including shift reports, incident reports, and behavioral observations.Maintain professional boundaries while building therapeutic rapport with clients.Participate in team meetings, shift handovers, and ongoing training to enhance skills and service delivery.Qualifications and Skills:Required:Recognized qualification or ongoing studies in Counselling, Health & Social Work, or equivalent experience in client services within a rehabilitation center.Excellent organizational, administrative, and multitasking skills.Strong communication and customer relation skills with a professional telephone manner.Preferred:3+ years of experience in a similar role.Understanding of cultural context and expectations.Proficiency in data capturing software and knowledge of behavioral health.At Samarpan, you’ll work in a dynamic, supportive environment where your contributions directly impact clients’ recovery journeys.
